Vaccine Clinics

The vaccine clinic at the Community & Cultural Center administering the Pfizer vaccine is now taking walk-ins. Previously booked appointments will take precedence. Vaccines for the 12 to 15 year old age bracket will be available. Those under 18 are required to have a parent or guardian present. Please bring proof of age for those under 18. A birth certificate or any item with a birth date will be sufficient. The number of walk ins accepted is subject to the number of doses available from that day's supply.

Need a ride to your vaccine appointment? Call VTA! Dial 408-809-2124 to schedule your ride. This service is for anyone in need of transportation to get vaccinated, regardless of immigration status.

For all participants for any vaccination at any site:

  • Vaccinations are by appointment only. 
  • Be sure to complete, print, and bring the consent form to your appointment. 
  • Bring BOTH medical and prescription insurance card.
  • Please bring a photo ID to the appointment.
  • Wear short sleeve shirt if possible. 
  • Do NOT attend the clinic if you are feeling unwell or experiencing COVID symptoms.
